chore: close governance issues 0009-0012 and remove docs/notes/.gitkeep

Add missing H1 titles and ✅ markers to issues 0009–0012 (implementation
complete; manual verification pending and tracked in ROADMAP.md housekeeping).
Remove .gitkeep from docs/notes/ now that the directory has real content.
