chore: close governance issues 0009-0012 and remove docs/notes/.gitkeep

Add missing H1 titles and ✅ markers to issues 0009–0012 (implementation
complete; manual verification pending and tracked in ROADMAP.md housekeeping).
Remove .gitkeep from docs/notes/ now that the directory has real content.

## What to build
Four targeted updates to existing reference documents to reflect the governance layer's existence. All four are small edits; they are bundled because they share the same dependency (governance.md must exist first) and the same purpose (keeping reference documents accurate).
