docs: fix two more stale commit hashes in simplification audit

Findings 13 and 21 cited b4c3d5e and 4bbd8a5, both dangling (not
reachable from HEAD) -- an amend after ace2d66 changed the hashes of
the underlying commits to 6cfc357 and f5e4d0d without updating the
self-reference. Corrected both.
